The current edition (Vol 4 iss7 25th April) of the IET's Engineering and Technology magasine has some interesting articles but most particularly "hard lessons- ten reasons for ten disasters". I hadn't heard of "The Vasa" (but actually seems similar to "The Mary Rose" only more so!) but generally (in)famous accidents from different fields of engineering.

None of them may appear directly relevant to railway signalling, but there are some common factors and thus lessons that can be drawn from most; after all each contains one common component- the human! More importantly it shows how the essential details of what went wrong and the underlying causes etc can be summarised in relatively few words- something to aim for when tackling exam questions.
