I have been made aware of this RSSB Publication-
Safety Manangement System Principles- moving beyond compliance which is freely downloadable from their website:

It states copyright 2011, but given the picture of the Olympc Rings at St Pancras international shown on page 67 it would appear to be more recent than that.
Certainly it would seem to be a very good source of what is effectively current information about the topic and hence particularly useful for the Module 1 (and also I suspect the Module 7) paper of the IRSE exam.
Hence I would recommend study by those wishing to extend their understanding of the topics in the context of UK mainline railway and indeed in railway systems more generally.
